IMPORTANT NOTICE
We have a new appointment request process. All requests for GP or nurse appointments will need to be submitted via our online form. You will need to complete our online form, which is only available during our normal opening hours.
If you are unable to complete the form yourself and do not have a friend or family member who can assist you, our staff will be happy to help.
When you submit a request, our team will review the information you provide and decide the most appropriate next step. This might be a phone call, an in-person appointment, or advice sent to you directly. This means that all requests for GP or nurse appointments will need to be submitted via our online form, available on our website.
We understand that this is a change from how things have been done before, and some patients may find it difficult at first. Please be assured that we will support any patients who are unable to complete the online form themselves.
We are making this change as part of our commitment to the Modern General Practice model, which is required by NHS England. It will help us to make sure that every request is seen by the right person, in the right timeframe, and that we use our appointments as effectively as possible.

Booking Appointments:
To request an appointment during normal opening hours, please complete one of our appointment request forms below. A member of the team will use the information provided to determine the time frame you need to be seen in and the type of clinician most suitable for your care, you can also:
- use your NHS account (through the NHS website or NHS App) or Patient Access
- phone us on 01384 252 729 our reception team will help you to complete an online form if you need assistance
- visit the surgery and speak with a receptionist, the team will help and support you with your online form if required

Nurse or healthcare assistant appointments
You can request a routine appointment with a nurse or healthcare assistant up to 2 weeks in advance.

Get medical advice from a doctor or nurse
To ask a non-urgent medical question:

Pre-bookable appointments
The practice offers additional pre-bookable appointments on Monday and Wednesday evenings between 6:30pm to 7:45pm.
Your appointment
However you choose to contact us, we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by text or email
Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.
Cancelling or changing an appointment
To cancel your appointment:
- use your NHS account (through the NHS website or NHS App)
- using the GP online system: Patient Access
- phone us on 01384 252 729 during opening times
- reply CANCEL to your appointment reminder text message
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Extended access
In the Dudley and Netherton area practices are working in partnership to provide additional GP appointments.
If you need access to an evening or weekend non-urgent appointment, please call the practice on 01384 252 729 and speak to the receptionist at your registered practice during normal opening hours.
Your appointment may be with another practice in the Dudley and Netherton area and may not be at your registered practice.

If you need help with your appointment

- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.
To request a home visit, it’s helpful if you phone the surgery as soon after 8am as possible.
